What Personal Data do I collect?

I collect and keep your completed health questionnaires (including email, phone numbers, emergency contact number and information about your health). I collect and keep your completed client forms for KCR/massage (including email, phone numbers, address and information about your health). I also collect names and email addresses when customers opt in to receive my newsletter.

When do I collect information?

I collect information from new customers when they complete a health questionnaire/client form and also when people subscribe to my newsletter via my website.

How is it processed?

I read completed forms to be aware of medical or health conditions. Where you have opted to receive the newsletter, your name and email address is added to by subscription list.

Where is information sent/stored?

Your paper forms go nowhere, they are stored securely in my home. Digital forms are stored securely on a google form. Emails are entered into a secure subscriber list. My phone has a PIN so no one else can access my phone to find your personal data.

How long do I hold your personal data?

The recommendation from my public liability insurance provider is that I keep records for a minimum of 6 years.

Anti-Spam Laws

The EU Anti-Spam laws set out the rules for direct commercial marketing, establishes requirements for commercial messages, gives recipients the right to have emails stopped from being sent to them, and spells out tough penalties for violations.

In accordance with UK Anti-Spam Laws and the GDPR Regulations, I agree to:

• NOT use false or misleading subjects or email addresses.

• Include the physical address of my business or site headquarters.

• Honour opt-out/unsubscribe requests promptly.

• Allow users to unsubscribe by using the link at the bottom of each email.
